Sehat Card — Stepwise Admission Instructions

Please follow these instructions and bring the required documents to ensure a smooth and trouble-free admission.

Make sure your Form B or CNIC is visible and clear

Verify your ID (Form B or CNIC)

Make sure your Form B (for minors) or CNIC is clear, readable, and not folded or blurred. Hospital staff will use it for identity verification.

Tip: Place the document on a clean flat surface and remove any covers for a clear scan or photo.

Verify patient age

If the patient is under 18, provide the patient's Form B to the Health Facilitator Officer to avoid any admission delays.فارم بی ہیلتھ فیسلیٹیٹر افسر کو فراہم کریں۔

Important: Form B is mandatory for patients under 18 years of age.

Patients aged 18 or above

If the patient is 18 years or older, you must present the Pakistani National Identity Card (CNIC) for the admission process.پاکستانی شناختی کارڈ (CNIC) لازمی طور پر دکھائیں۔

If you have paid for any tests or investigations yourself, keep all receipts safe. They will be required at discharge for reimbursement.

اگر آپ نے کسی بھی ٹیسٹ یا لیب انویسٹی گیشنز کی رقم خود ادا کی ہے تو تمام رسیدیں محفوظ رکھیں۔ چھٹی کے وقت ری ایمبرسمنٹ کے لیے ان کی ضرورت پڑے گی۔

Store receipts in a safe place such as an envelope inside your pocket or bag.

Bring original documents

Bring your original Form B or CNIC for the admission process. Hospital staff will scan these documents and return the originals to you.

Note: Original documents are returned after scanning — please keep them safe.
